Which Direct Deposit Form applies to me?
State Employees that want to begin receiving
their paycheck or travel reimbursement(s) electronically, download the eVendor

 Direct Deposit Authorization for Payroll and Travel
Vendors who would like to receive their
payments electronically from the State of West Virginia will have to download
the eVendor Agreement for Companies.

Retirees who need to start or change their
direct deposit must download the Direct Deposit Form for Retirement.

DHHR Birth to Three participants who would
like to receive their payments electronically will have to download the eVendor
Agreement for Birth to Three. Birth to Three participants will not receive an
email notification.

Counties, Municipalities & Volunteer Fire Departments
that receive severance taxes or surcharges and want to setup or change their
direct deposit must download and complete a Direct Deposit Authorization.

FACTS Providers who want to start or change
their direct deposit will need to download the FACTS eVendor Agreement.

Administrators who need to have a payment
removed from the direct deposit file must download and complete the Direct
Deposit Pull-Off Form.
Administrators who need to have a direct deposit payment returned must download
and complete the Direct Deposit Reversal Form.

State Agencies that have a vendor receiving
a single nonrecurring payment electronically which is paid by an Off System
document will have to download the eVendor Agreement for Off System Payments.
