 Direct Deposit Authorization for Payroll and Travel
Vendors who would like to receive their payments electronically from the State of West Virginia will have to download the eVendor Agreement for Companies.

Retirees who need to start or change their direct deposit must download the Direct Deposit Form for Retirement.

DHHR Birth to Three participants who would like to receive their payments electronically will have to download the eVendor Agreement for Birth to Three. Birth to Three participants will not receive an email notification.
